It's always interesting to speculate how things would have gone had John made a different decision at any of several turning points. This one points out that Dean had been in the hunting life at a very formative time in his childhood, and that he couldn't just walk away from it and believe in bright lights and "normal" anymore. Sam was young enough to slough it off and accept their life at surface value. What hurts the most, I think, is that in this AU, John is still riding Dean, making it his responsibility that unwanted knowledge clings to Dean, that he's unable to unknow what he knows, and further, blaming Dean for it. Once again, Dean is made to suffer for Sam's sake.
At least he's vindicated in the end.
