Yeah, you're totally right. Because now, with this episode, there's a dichotomy between 'good' incest and 'bad' incest, and even the good incest isn't really that good, huh? we're either supposed to understand that other people are --unfortunately -- expendable in the quest of one brother saving another (the 'good' incest committers) or that Sam and Dean's incestuousness isn't as good as we think it is. so even our protagonists are cast into the shady area.

Maybe that's the YED's point? Maybe he's playing on something he picked up about Dean. It's like, the ultimate anathema has a completely different color to it, in light of what canon's given us so far. Instead of being just horrified by Azazel's taunts, we get this additional, mirroring layer that demonstrates the distinction?
